ATHOL, ID – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Merritt Bros. Lumber Company Inc. for exposing employees to multiple workplace safety hazards at its Athol facility. The Athol, Idaho-based company is facing $189,221 in proposed penalties.

ORANGE CITY, FL – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) cited Pavemax Corp. for safety violations after an employee suffered fatal injuries at an Orange City worksite.

OSHA inspectors determined that a paver operator lacked proper training to use the equipment, and was allowed to stand on the equipment while it was in motion, and subsequently fell and was pulled under the paver.

PORTLAND, OR – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Vigor Marine Inc. for 16 willful and serious violations following an inspection at the shipbuilder's Portland, Oregon, facility. The company faces proposed penalties totaling $370,358.

The company faces proposed penalties of $507,374, and OSHA has placed Gavilon Grain LLC in the Agency's Severe Violator Enforcement Program.

DALLAS, TX – The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) and the Manhattan Construction Company recently signed a partnership to help protect approximately 1,000 workers during construction of Globe Life Field in Arlington. DALLAS, TX – U.S. District Court Judge Ed Kinkeade has ordered Design Plastering West LLC to pay criminal and civil penalties for criminal violations of occupational safety and health standards after an employee suffered a fatal fall at an apartment complex in Dallas. The court ordered the company to pay a $150,000 criminal fine, $100,000 civil penalty, admit to eight willful violations, and to undergo monitoring by the Occupational Safety and Health Administration (OSHA) for four years.

NEW CASTLE, DE – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Wilmington Fibre Specialty Company Inc. for exposing employees at its New Castle, Delaware, facility to workplace safety hazards. The company faces $146,152 in proposed penalties.

OPELIKA, AL – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Packaging Corp. of America for exposing employees to unprotected machinery and amputation hazards. The cardboard box manufacturer, based in Opelika, Alabama, faces proposed penalties of $66,612.

LAKEWOOD, NJ - The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited GMG Brito Contractors LLC for exposing employees to fall and other safety hazards at a residential construction site in Lakewood, New Jersey. OSHA has proposed penalties totaling $127,436.

CALVERT CITY, KY – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited five contractors for safety and health violations after three employees were fatally injured and two others critically injured following an explosion onboard a towboat in Calvert City, Kentucky. The five companies collectively received 55 of violations with proposed penalties totaling $795,254.

APPLETON, WI ‒ The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) cited Carlos Ketz, who operates as Ketz Roofing, for the sixth time in the past five years for exposing employees to falls. OSHA proposed penalties totaling $48,777. 

OSHA inspectors cited the roofing contractor – based in Menasha, Wisconsin – after observing employees working without adequate fall protection on a commercial building in Appleton.

LEBANON, MO – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Karrenbrock Excavating LLC for allowing two employees to work in an unprotected trench while installing sewers. The company faces proposed penalties of $189,221.

SYRACUSE, NY – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Timberline Hardwood Floors LLC for willful and serious violations of workplace safety and health standards. The Fulton, New York, custom hardwood-flooring manufacturer faces proposed penalties totaling $182,917.

HOMERVILLE, GA – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Dupont Yard Inc. after an employee suffered a partial hand amputation and other injuries while working on unguarded machinery in Homerville, Georgia. The wooden post manufacturer faces $109,548 in proposed penalties.

BELHAVEN, NC – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) has cited Belhaven Shipyard and Marina Inc. - doing business as TowBoatUS River Forest - after an employee drowned when a towboat capsized while operating in a winter storm. The company faces $11,640 in proposed penalties.

ST. LOUIS, MO – The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) and Alberici Healthcare LLC in St. Louis, Missouri, have signed a strategic partnership to promote worker safety and health during the construction of the 802,000-square-foot SSM Health Saint Louis University Hospital.

The partnership will address falls, struck-by, caught-in or between, and electrocution hazards, the top four causes of fatalities in construction.
