ERISA Reporting Forms

EFAST2 is an all-electronic system designed by the U.S. Department of Labor, Internal Revenue Service, and Pension Benefit Guaranty Corporation to simplify and expedite the submission, receipt, and processing of the Form 5500 and Form 5500-SF. These forms must be electronically filed each year by employee benefit plans to satisfy annual reporting requirements under the Employee Retirement Income Security Act (ERISA) and the Internal Revenue Code. Under EFAST2, filers choose between using EFAST2-approved vendor software or this website (IFILE) to prepare and submit the Form 5500 or Form 5500-SF. Completed forms are submitted via the Internet to EFAST2 for processing.

Filing Searches

The Form M-1 Online Search tool allows users to find reports for Multiple Employer Welfare Arrangements (MEWAs) and Association Health Plans (AHPs) filed with the U.S. Department of Labor. The Form M-1 is filed annually by MEWAs (including AHPs) and ECEs (Entities Claiming Exception). Copies of the Form M-1 filings are also available from EBSA's Public Disclosure Room at 200 Constitution Ave NW, Room N-1515, Washington, DC 20210, or by calling 202-693-8673.

These two online search tools allow users to search and view Apprenticeship and Training Plan Notices or Top Hat Plan Statements for certain pension plans maintained by an employer for a select group or management or highly compensated employees filed with the Department of Labor. Results can be printed or downloaded to Excel.
