Dear Mr. Cassity:

This office has recently completed an audit of UTU SLB 20 under the Compliance Audit Program (CAP) to determine your organization’s compliance with the provisions of the Labor-Management Reporting and Disclosure Act of 1959 (LMRDA). As discussed during the exit interview with you that was performed via telephone on June 13, 2012, the following problem was disclosed during the CAP. The matters listed below are not an exhaustive list of all possible problem areas since the audit conducted was limited in scope.

Reporting Violation

Title II of the LMRDA requires the president and treasurer or corresponding principal officers of each labor organization to file a Form LM-2, 3 or 4 Labor Organization Annual Report with the Office of Labor-Management Standards (OLMS) within 90 days after the end of each fiscal year. Prior to the CAP, OLMS had not received a report from SLB 20 for fiscal year ending (FYE) 12/31/2011. However, you filed the FY 2011 report via the OLMS electronic filing system (EFS) on June 13, 2012. Therefore, this violation has been resolved.
