The Division of Energy Employees Occupational Illness Compensation (DEEOIC) is announcing the release of a newly consolidated form to file a claim for impairment or wage loss, the EE-10. The EE-10 replaces the current EN-10, EN-11A, and EN-11B. The new EE-10 will be available for use beginning June 1, 2025.

We are in the process of updating our Energy Document Portal (EDP), which has yet to be updated with the new EE-10. In the interim, claimants may continue to use the EN-10, EN-11A and EN-11B when submitting documents via the portal. Documents submitted within the EDP are available to DEEOIC claims staff immediately after the document upload is complete, thus eliminating mailing delays. EEOICPA stakeholders can find more information and access the EDP by visiting the EDP website.

Use of the EDP is voluntary. If claimants do not wish to electronically file a claim or upload documents into the EDP, claimants can find the new EE-10 and other claim forms located at the Energy forms page.

Claimants should include the DEEOIC Case ID number on all documents mailed to the CMR.

Claimants also have the option of visiting one of our 11 Resource Centers to have staff answer questions about the EE-10 or assist them in filing their claim for impairment or wage loss.
