Background: Copies of the Quality Appraisal results for FY 1993 are being transmitted under separate cover to all States. This report contains the results of all State Employment Security Agencies (SESAs) for all performance measures in the Quality Appraisal system. Measures in the Quality Appraisal system, using data reported by SESAs, include first benefit payment promptness, appeals promptness, field audit penetration, report delinquency, cash management and benefit payment control. Measures using results from on-site reviews performed by SESAs or Federal staff include nonmonetary determinations performance and promptness, appeals performance, employer status determinations, field audit performance and collections. The quantifiable goals SESAs should achieve in the various measures are designated as Secretary's Standards (SS) and Desired Levels of Achievement (DLAs). Criteria in the Secretary's Standards apply to intrastate and interstate first payment promptness and lower authority appeals promptness. Action Required: SESA Administrators are requested to: a. Review the FY 1993 Quality Appraisal report and identify any activities not meeting the designated SS/DLAs; and b. For those activities not meeting the SS/DLAs, submit corrective action plans in accordance with the FY 1994 Program and Budget Planning (PBP) instructions.

AWBA Utilization: The attached listing identifies the AWBA which is to be used when computing the weekly amount of disaster unemployment assistance (DUA) for major disasters where the date of the disaster for the individual occurs during the fourth quarter of FY 1993 (refer to 20 CFR 625.6). The AWBA for each State is based on the total amount of unemployment insurance paid in that State in the first four of the last five completed calendar quarters immediately preceding the quarter in which the major disaster began. States shall use the AWBA, rounded to the next higher dollar amount, in the computation of the DUA weekly benefit amount. An updated listing will be provided for each subsequent calendar quarter. Action Required: State Employment Security Agency Administrators are requested to provide this information to appropriate staff.

Background: Due to changes in personnel, facilities and agency reorganizations, periodic updates of State agency contacts are necessary. States have provided the names and telephone numbers of individuals designated to respond to inquiries from other States about UC tax and employer accounts. This directory reflects information provided through June 1993. It replaces the directory attached to UIPL No. 35-92. Changes: When changes occur, the page to be changed should be copied and the changes penned in. Mail or fax the revised page to the appropriate Regional Office. They will update their information and forward the changes to the Unemployment Insurance Service (UIS) National Office for inclusion in the next directory update. Action Required: All SESAs are requested to ensure that a copy of the attached directory is made available to appropriate personnel.

Reference: UIPL No. 3-91, Change 1, dated January 22, 1991. Background: The new UI National Office FAX number for forwarding advance information on Title XII loans and repayments is now 202-219-8506, located in room C-4512 of the Frances Perkins Building. In the event that the FAX equipment is inoperable or inaccessible, backup capability is available in ORM in the Employment and Training Administration on 202-219-6510. The new numbers are effective immediately. After the FAX message is transmitted, a confirming telephone call must be immediately made to TEUMI, 202- 219-5216, advising of the existence of the FAX message, date and time sent, where it was sent, loan and/or repayment amount, and effective date as appropriate. This will alert appropriate staff and trigger immediate followup to acknowledge receipt for processing. Official documentation, signed by the Governor or a designated individual, must be forwarded to TEUMI as soon as possible. 4. Action Required. SESAs should include these FAX numbers on their list of significant telephone listings.
