SUBJECT	:	Petition for NAFTA Transitional Adjustment Assistance Form Including a Spanish Version

1. Purpose. To provide copies of ETA Form 9042, Petition for NAFTA Transitional Adjustment Assistance, and the Spanish version, ETA Form 9042-1, Solicitud de Asistencia Para Ajuste Ocupacional Bajo NAFTA.

2. OMB Approval. ETA Form 9042 and ETA Form 9042-1 are approved by the Office of Management and Budget (OMB) in accordance with the Paperwork Reduction Act of 1980; OMB approval number 1205-0342; expiration date 06/30/2001.

3. Action Required. State Administrators are requested to distribute these approved forms to appropriate State staff.

4. Inquiries. Inquiries should be directed to the appropriate Regional Office.
