DENVER– An U.S. Department of Labor’s Occupational Safety and Health Administration investigation found a Denver company violated federal law when it terminated an employee for insubordination after the worker reported safety concerns about a product being manufactured by the company.

CAMBRIDGE, Ohio – For the second time in two years, federal safety inspectors have found Shelly & Sands Inc. – a large regional highway construction company – exposing its workers to the serious and sometimes fatal risk of falls by allowing them to climb over guardrails of an aerial lift onto a highway overpass.

CINCINNATI, Ohio – Just five weeks after a 28-year-old maintenance worker lost part of his right arm in an improperly guarded bread wrapping machine at the Cincinnati-based Klosterman Baking Co., federal safety inspectors investigating the injury found another worker exposed to the same hazard.

WEST POINT, Neb. – Working to clear crusted corn from the sides of a grain bin, a 52-year-old maintenance employee found himself engulfed in hundreds of pounds of grain, just minutes after the wall of corn collapsed and buried him. Rescued by emergency crews, he died of his injuries two days later.

JEFFERSON CITY, Mo. – Federal investigators have cited a Jefferson City roofing contractor after a 47-year-old laborer collapsed on Aug. 17, 2016, and later died after being hospitalized with a core body temperature above 107 degrees.

MORAINE, Ohio – Workers began making multiple complaints of unsafe working conditions at the Fuyao Glass America Inc. facility in Moraine not long after it opened in October 2015, leading to eight separate federal inspections and $226,937 in proposed penalties for the automotive glass supplier to plant brands such as Audi, Cadillac, Land Rover, Volvo and Volkswagen.

HUDSON, S.D. – Federal investigators found multiple safety violations at a South Dakota ethanol refinery expansion project after a 38-year-old pipefitter suffered fatal burn injuries when ethanol spilled from a process pipe he was working on and was ignited by flames from nearby welding operations.

DALLAS – For manufacturing industry workers, amputation is serious risk and the cause of more than a 1,400 serious injuries each year. In 2015, the U.S. Department of Labor’s Occupational Safety and Health Administration received reports of more than 2,600 amputations nationwide – 57 percent of them suffered by manufacturing workers.

Most often, amputations occur when workers operate machines without proper or adequately safety guards.

PATASKALA, Ohio – For at least two years, an Ohio manufacturer of thermoplastic lining systems for truck bodies and trailers exposed workers to the risk of amputation and other serious injury by disabling safety devices to speed production.

MINERAL WELLS, Texas – While investigating one serious employee injury in May 2016, at PECOFacet – a global manufacturer of filtration and containment products – inspectors with the U.S. Department of Labor’s Occupational Safety and Health Administration found themselves investigating a second incident in which another employee was hurt seriously three days after they arrived.

PLAINFIELD, Ill. – A federal investigation prompted by the death of a 50-year-old worker at the Plainfield steel processing facility has resulted in a half-dozen safety and health violations.

The U.S. Department of Labor’s Occupational Safety and Health Administration has issued six serious safety and health violations to Coilplus Illinois Inc., for violations its inspector found after the June 23, 2016, death.
