Office of Labor-Management Standards (OLMS)
OLMS has disabled functionality for downloading blank forms using the Adobe forms system.
Labor organizations can now acquire and complete Form LM-2, LM-3, and LM-4 via the new OLMS Electronic Forms System (EFS).
Access the OLMS EFS here to register for the system and obtain, work on, or sign and submit your Form LM-2, LM-3, or LM-4 report.
Please note: Filers will be able to complete, digitally sign, and submit Adobe-based Form LM-2, LM-3, and LM-4 reports already in progress via the Upload Form page until July 31, 2011, at which time the transition to the EFS system will be complete.
