DOWNLOAD OLMS FORMS LM-2, LM-3, LM-4 (Informed Filler Version) Installation Instructions Please Note: If you have an old version of the Informed Filler software on your computer, you must uninstall it and reboot your computer before installing the new version. 1. Download Electronic Forms LM-2, LM-3, LM-4 Software Version 3.0 (.exe) (11 mb) 2. Select the "Save" option and save the file to your desktop. 3. Double click the "OLMS3" file. It is recommended that you quit all other programs before running the installation. 4. When the system prompts you to enter a Filler Registration Key, simply click "Next". A dialogue box displays, asking you to confirm that you are using Keyed Forms and do not need the Registration Key. Click "Yes" to continue, and follow the instructions on screen to complete installation. Please note: For reports for fiscal years beginning before July 1, 2004, the simplified annual report Form LM-3 may only be used by unions with total annual receipts of less than $200,000, if not in trusteeship. For fiscal years beginning July 1, 2004 or later, Form LM-3 may be used by unions with less than $250,000 in total annual receipts, if not in trusteeship. This Form LM-2 may only be used for fiscal years beginning before July 1, 2004. For fiscal years beginning on July 1, 2004 or later, LM-2 filers must use the electronic Revised Form LM-2 located here. If you encounter problems downloading or installing the Electronic Forms Package, call the DOL Help Line at 1-866-487-2365 for assistance.
