LS-275ic, Agreement and Undertaking (Insurance
Carrier) and LS-275si, Agreement and
Undertaking (Self-Insured Employer) that following the
new regulations are
now required, replacing the OWCP-1 (Agreement and Undertaking) for Longshore.
