Enrollment Notices Under WHCRA
Plans must also provide notices describing the benefits required under WHCRA upon enrollment in the plan.
The enrollment notice must describe the benefits that WHCRA requires the group health plan to cover. Additionally, the enrollment notice must describe any deductibles and coinsurance limitations applicable to such coverage. (Under WHCRA, coverage of the required benefits may be subject only to deductibles and coinsurance limitations consistent with those established for other medical/surgical benefits under the plan or coverage.)
The Department has developed a model enrollment notice that may be helpful. Correct use of the model will generally assure compliance with the statutory requirements.
